Forms in Way We Do enables you to create web-based forms for team members to complete to store structured data as part of your organization’s Management Systems, and governance and compliance programs.
Creating the Form
You can create a Form in one of two ways.
Method 1: Select Manual, then select Create a Form.
Method 2: From within a Label, select +Add New, then choose Create a Form.
Give your Form a title.
Click the plus icon to add form fields and content.
Form Question Types
We have fourteen (14) Form field types that can be used within Forms. Together you can create the type of Form you need to store structured data as a Record.
Add a short text field
To add a short text field, select the plus button and choose Add a short text field.
This will add in a short text field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions about how to complete the form field (optional).
Short text field settings
Select the cog icon.
The text field will flip to show the settings available.
Modify the options as required or use the default settings:
Max Length - by default, the maximum number of characters that can be entered is 255. You can reduce or increase this number.
Placeholder Text - by default, there is no placeholder text. You are able to add an example of text to demonstrate to the user how to complete the form field. When the user clicks on the form field, the placeholder text is removed.
Default Value - by default, there is no default text displayed. You are able to add a default value which can be text, characters or numbers. It is pre-populating the field which means the user can modify it or leave it as is when they complete the form.
Required - by default, the field is not required to be completed by the user. Click the toggle to change it to Yes if the form field is mandatory.
Add a long text field
To add a long text field, select the plus button and choose Add a long text field.
This will add in a long text field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).
Long text field settings
Select the cog icon.
The text field will flip to show the settings available.
Modify the options as required or use the default settings:
Max Length - specify the maximum number of characters allowed for longer responses. The default maximum length is 512 characters.
Placeholder Text - add an example paragraph to guide the user's response.
Default Value - pre-populate the field with standard text if needed.
Required - toggle to Yes if the form field is mandatory.
Add a number field
To add a number field, select the plus button and choose Add a number field.
This will add in a number field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).
Number field settings
Select the cog icon.
The number field will flip to show the settings available.
Modify the options as required or use the default settings:
Decimal Places - define how many decimal places are permitted.
Minimum/Maximum Value - restrict the number range the user can enter.
Placeholder Text - provide an example number format.
Default Value - pre-populate a specific number.
Required - toggle to Yes if the form field is mandatory.
Add a currency field
To add a currency field, select the plus button and choose Add a currency field.
This will add in a currency field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).

Select the cog icon.
The currency field will flip to show the settings available.
Modify the options as required or use the default settings:
Currency Symbol - enter the appropriate currency marker (e.g., $, €, £).
Decimal Places - define how many decimal places are permitted.
Minimum/Maximum Value - restrict the numerical range allowed.
Placeholder Text - provide an example monetary amount.
Default Value - pre-populate a standard amount.
Required - toggle to Yes if the form field is mandatory.
Add a percentage field
To add a percentage field, select the plus button and choose Add a percentage field.
This will add in a percentage field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).
Percentage field settings
Select the cog icon.
The percentage field will flip to show the settings available.
Modify the options as required or use the default settings:
Decimal Places - define how many decimal places are allowed.
Minimum/Maximum Value - restrict the percentage range (e.g., 0 to 100).
Placeholder Text - provide an example percentage format.
Default Value - pre-populate a starting percentage.
Required - toggle to Yes if the form field is mandatory.
Add a URL field
To add a URL field, select the plus button and choose Add a URL field.
This will add in a URL field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).
URL field settings
Select the cog icon.
The URL field will flip to show the settings available.
Modify the options as required or use the default settings:
Placeholder Text - provide an example web address (e.g., https://www.example.com).
Default Value - pre-populate a standard web address.
Required - toggle to Yes if the form field is mandatory.
Add an email field
To add an email field, select the plus button and choose Add an email field.
This will add in an email field block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).

Select the cog icon.
The email field will flip to show the settings available.
Modify the options as required or use the default settings:
Placeholder Text - provide an example email (e.g., name@company.com).
Default Value - pre-populate a standard email address.
Required - toggle to Yes if the form field is mandatory.
Add a phone number field
To add a phone number field, select the plus button and choose Add a phone number field.
This will add in a phone number field block, where you will format it.
Click Untitled Field to enter the field title
Use the Add a description section to provide further instructions (optional).

The phone number field will flip to show the settings available.
Modify the options as required or use the default settings:
Required - toggle to Yes if the form field is mandatory.
Add a single-select field
To add a single-select field, select the plus button and choose Add a single-select field.
This will add in a single-select block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).

Modify the options as required or use the default settings:
Data Source: create your own or choose a predefined list of options for the user to select from.
Custom Options - enter the list of choices the user can select from, one per line.
Company Roles - predefined list of roles from the Org Chart in the Structure section of Way We Do
Countries - predefined list of countries
Procedures - the ability to select procedure titles
Timezones - predefined list of timezones
Users- predefined list of user names from the Team Management section of Way We Do
Yes/No - predefined Yes/No selection list

Add a multi-select field
To add a multi-select field, select the plus button and choose Add a multi-select field.
This will add in a multi-select block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).

Modify the options as required or use the default settings:

Add a date/time field
To add a date/time field, select the plus button and choose Add a date/time field.
This will add in a date/time block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).

The date/time field will flip to show the settings available.
Modify the options as required or use the default settings:
Date Type - choose between either Date Only, Date & Time, or Time Only
Default to Today - toggle to Yes if today's date is to display automatically.
Use Timezones - toggle to Yes to automatically convert the entered date and time into the viewer's own timezone.
Placeholder Text - provides space for additional instructions (e.g., Enter date).
Required - toggle to Yes if the form field is mandatory.
Add an attachments field
To add an attachments field, select the plus button and choose Add an attachments field.
This will add in an attachments block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).
Attachments field settings
Select the cog icon.
The attachments field will flip to show the settings available.
Modify the options as required or use the default settings:
Max Attachments Allowed - limit how many files can be attached to this specific field.
Images Only - restrict uploads to image files only (e.g. PNG, JPG, GIF)
Required - toggle to Yes if the form field is mandatory.
Add a formula field
To add a formula field, select the plus button and choose Add a formula field.
This will add in a formula block, where you will format it.
Click Untitled Field to enter the field title.
Use the Add a description section to provide further instructions (optional).

Select the cog icon.
The formula field will flip to show the settings available.
Modify the options as required or use the default settings:
Expression - configure the calculation by selecting previously created number, currency, or percentage fields. Enter $ and first few letters of the field names to display them in a list to select from. For further assistance, view the dedicated support article on Formula Expressions.
Hidden - to hide the Formula field on the form when in View or Submission Mode. The Formula output is available in the Register upon submission of the Form.
Use Timezones - toggle to Yes to automatically convert the entered date and time into the viewer's own timezone. Placeholder Text - provides space for additional instructions (e.g., Enter date).
Format - determine the format of the output (e.g. 0.00)
Add a content field
To add a content field, select the plus button and choose Add a content field.
This will add in a content block, where you can add your content and format it.
Use the options as required:
Rich Text Content - use this area to add instructional text, disclaimers, or reference material directly into the body of the form without requiring user input.
Formatting Options - utilize standard text formatting tools (headings, bold, italics, etc.) to ensure your instructions are clear and easy to read.
